De los 1.407 pat\u00f3genos que afectan a los seres humanos, el 58% son de origen animal, una cuarta parte de los cuales puede ser una posible fuente de transmisi\u00f3n de una epidemia o pandemia, como los\u00a0 virus de la influenza\u00a0 y el \u00c9bola\u00a0.\u00a0Adem\u00e1s, el\u00a0 75% de las enfermedades infecciosas emergentes\u00a0son de origen animal. Una mejor comprensi\u00f3n y gesti\u00f3n de los reservorios animales de agentes infecciosos, pero tambi\u00e9n de sus v\u00edas de transmisi\u00f3n y adaptaci\u00f3n a los seres humanos, es fundamental para el control de las zoonosis\u00a0y las futuras epidemias.<\/p>\n<hr \/>\n<p>It is both a concept, a strategy, and a goal. \u201cOne Health\u201d has gradually established itself in the life sciences, veterinary medicine, and biomedical sciences. It now dominates the communication of international public health organisations such as the <a href=\"https:\/\/www.oie.int\/en\/for-the-media\/onehealth\/\" target=\"_blank\" rel=\"noopener noreferrer\">World Organization for Animal Health<\/a> (OIE), the <a href=\"http:\/\/www.fao.org\/3\/al868e\/al868e00.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Food and Agriculture Organization<\/a> (FAO), the <a href=\"https:\/\/www.who.int\/news-room\/q-a-detail\/one-health\" target=\"_blank\" rel=\"noopener noreferrer\">World Health Organization<\/a> (WHO) and <a href=\"https:\/\/www.cdc.gov\/ncezid\/what-we-do\/2018-highlights\/one-health.html\" target=\"_blank\" rel=\"noopener noreferrer\">the Centre for Disease Control and Prevention<\/a> (CDC).<\/p>\n<p>However, \u201cOne Health\u201d remains little known to the public and is rarely integrated into government decision-making process. Representing a real paradigm shift, could help us better understand, anticipate, and manage the outbreak of new pandemics.<\/p>\n<p id=\"h-the-obvious-interconnection-of-living-things\"><strong>The obvious interconnection of living things<\/strong><\/p>\n<p>The term \u201cone medicine\u201d was <a href=\"https:\/\/books.google.fr\/books\/about\/Veterinary_medicine_and_human_health.html?id=fJqFAAAAIAAJ&amp;redir_esc=y\" target=\"_blank\" rel=\"noopener noreferrer\">introduced in 1984<\/a> by Calvin Schwabe. The American epidemiologist intended to underline the obvious interconnection between animal and human medicine. But he also pointed out the urgent need for collaboration between veterinarians and doctors to control the spread of infectious agents.<\/p>\n<p>Of the 1,407 pathogens affecting humans, <a href=\"https:\/\/wwwnc.cdc.gov\/eid\/article\/11\/12\/05-0997_article\" target=\"_blank\" rel=\"noopener noreferrer\">58% are of animal origin<\/a>, a quarter of which can be a possible transmission source for an epidemic or pandemic, like the <a href=\"https:\/\/en.wikipedia.org\/wiki\/Influenza\" target=\"_blank\" rel=\"noopener noreferrer\">influenza<\/a> and <a href=\"https:\/\/en.wikipedia.org\/wiki\/Ebola\" target=\"_blank\" rel=\"noopener noreferrer\">Ebola<\/a> viruses. In addition, <a href=\"https:\/\/royalsocietypublishing.org\/doi\/10.1098\/rstb.2001.0888\" target=\"_blank\" rel=\"noopener noreferrer\">75% of emerging infectious diseases<\/a> are of animal origin. A better understanding and management of animal reservoirs of infectious agents, but also their routes of transmission and adaptation to humans, is therefore essential for the control of <a href=\"https:\/\/en.wikipedia.org\/wiki\/Zoonosis\" target=\"_blank\" rel=\"noopener noreferrer\">zoonosis<\/a> and future epidemics. Hence the growing interest in <a href=\"https:\/\/en.wikipedia.org\/wiki\/EcoHealth\" target=\"_blank\" rel=\"noopener noreferrer\">EcoHealth<\/a>, an emerging discipline that cuts across ecology, epidemiology, and biomedical sciences.<\/p>\n<p>It is disturbances in the dynamics of interactions between populations of humans, infectious agents, animal reservoirs, and sometimes <a href=\"https:\/\/en.wikipedia.org\/wiki\/Vector_(epidemiology)\" target=\"_blank\" rel=\"noopener noreferrer\">insect vectors<\/a>, which usually trigger epidemics of zoonotic origin. By varying their habitat or their abundance, environmental, climatic, and socio-economic changes can, for example, modify the probabilities of interactions between each population. Furthermore, infectious agents, and in particular <a href=\"https:\/\/en.wikipedia.org\/wiki\/RNA_virus\" target=\"_blank\" rel=\"noopener noreferrer\">RNA viruses<\/a>, evolve very rapidly. They can therefore adapt to new hosts if they come into frequent contact with them, thus creating a new network of interactions.<\/p>\n<p id=\"h-hunting-deforestation-climate-and-epidemics\"><strong>Hunting, deforestation, climate\u2026 and epidemics<\/strong><\/p>\n<p>The link between human intrusion into an ecosystem and the onset of an epidemic is well illustrated by the case of the <a href=\"https:\/\/en.wikipedia.org\/wiki\/HIV\" target=\"_blank\" rel=\"noopener noreferrer\">human immunodeficiency virus<\/a> (HIV), which killed more than 32 million people between 1981 and 2018. Its emergence is probably due to an increase in hunting and consumption of chimpanzee meat in the Kinshasa region (Democratic Republic of the Congo) <a href=\"https:\/\/science.sciencemag.org\/content\/346\/6205\/56\" target=\"_blank\" rel=\"noopener noreferrer\">in the years 1920-50<\/a>: the increased contact between humans and primates infected with the <a href=\"https:\/\/en.wikipedia.org\/wiki\/Simian_immunodeficiency_virus\" target=\"_blank\" rel=\"noopener noreferrer\">simian immunodeficiency viruses<\/a> have promoted adaptation of this pathogen to humans.<\/p>\n<p><a href=\"https:\/\/en.wikipedia.org\/wiki\/Lyme_disease\" target=\"_blank\" rel=\"noopener noreferrer\">Lyme disease<\/a> can also be cited as an example. This pathology, which testifies to the <a href=\"https:\/\/journals.plos.org\/plosmedicine\/article?id=10.1371\/journal.pmed.0030231\" target=\"_blank\" rel=\"noopener noreferrer\">links<\/a> between <a href=\"https:\/\/www.nature.com\/articles\/nature09575\">alteration of biodiversity and epidemics<\/a>, is due to a bacterium, <em>Borrelia burgdorferi<\/em>, transmitted by tick bites. In nature, ticks feed on many vertebrates. Some of these, such as squirrels and deer, are quite resistant to infection. Others, such as mice, are very susceptible to it. Because of the <a href=\"https:\/\/fr.wikipedia.org\/wiki\/Dilution_(%C3%A9co%C3%A9pid%C3%A9miologie)\" target=\"_blank\" rel=\"noopener noreferrer\">dilution effect<\/a>, there are thus few infected ticks in forests with high biodiversity. But where it is low, in small wooded plots where predators are few, mice can see their numbers increase, which increases the frequency of tick infection and the risk for humans: in the Northeastern United States and Europe, a historic cycle of deforestation, reforestation and fragmentation of wooded areas has encouraged the progression of the disease.<\/p>\n<p>The last example: global warming. It is now well established that it is a game changer for a <a href=\"https:\/\/academic.oup.com\/femsle\/article\/365\/2\/fnx244\/4631076\" target=\"_blank\" rel=\"noopener noreferrer\">wide range of vector-borne diseases in Europe<\/a> and will continue to do so in the decades to come. We know, for example, that the tiger mosquito of Asian origin (<a href=\"https:\/\/en.wikipedia.org\/wiki\/Aedes_albopictus\" target=\"_blank\" rel=\"noopener noreferrer\"><em>Aedes albopictus<\/em><\/a>), vector of diseases such as <a href=\"https:\/\/en.wikipedia.org\/wiki\/Zika_virus\" target=\"_blank\" rel=\"noopener noreferrer\">Zika<\/a>, <a href=\"https:\/\/en.wikipedia.org\/wiki\/Dengue_fever\">dengue<\/a> and <a href=\"https:\/\/en.wikipedia.org\/wiki\/Chikungunya\">chikungunya<\/a>, or the sand fly (<a href=\"https:\/\/en.wikipedia.org\/wiki\/Phlebotominae\"><em>phlebotominae<\/em><\/a>), native to the Mediterranean basin and North Africa and which transmits <a href=\"https:\/\/en.wikipedia.org\/wiki\/Leishmaniasis\" target=\"_blank\" rel=\"noopener noreferrer\">leishmaniasis<\/a>, have now established themselves in southern Europe.<\/p>\n<p id=\"h-management-dependent-on-the-societal-context\"><strong>Management dependent on the societal context<\/strong><\/p>\n<p>To best manage an epidemic, it is important to consider the <a href=\"https:\/\/www.clinicalmicrobiologyandinfection.com\/article\/S1198-743X(14)63865-9\/fulltext\" target=\"_blank\" rel=\"noopener noreferrer\">socio-economic<\/a>, <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0277953614007199\" target=\"_blank\" rel=\"noopener noreferrer\">political<\/a>, <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0277953614007199\" target=\"_blank\" rel=\"noopener noreferrer\">religious, and cultural<\/a> realities of the country concerned. The support of populations for public health strategies is also essential. In short, communication and education strategies must adapt to each societal context.<\/p>\n<p>Take the case of <a href=\"https:\/\/en.wikipedia.org\/wiki\/Brucellosis\" target=\"_blank\" rel=\"noopener noreferrer\">brucellosis<\/a>. This disease is caused by <em>Brucella<\/em> bacteria, several species of which chronically infect domestic ruminants. Humans can be infected through direct contact with affected animals or, more often, through consumption of contaminated food, but transmission between humans is almost nonexistent. Acting on the animal reservoir thus reduces the economic costs associated with the loss of livestock and improves human health.<\/p>\n<p>In Europe, brucellosis has been virtually eradicated by vaccination and systematic killing of infected herds. This approach, however, has only been made possible by harmonizing the health policies of different countries \u2013 including the identification of domestic animals, their testing and control of their movements \u2013 and compensation for breeders. However, this cannot be applied to developing countries, where both economic resources and the operational capacities of veterinary services are limited. Take the case of India, where <a href=\"https:\/\/link.springer.com\/article\/10.1007\/s11250-013-0362-y\" target=\"_blank\" rel=\"noopener noreferrer\">brucellosis remains endemic<\/a>. The lack of financial compensation for farmers and the religious killing ban on cattle make it impossible to test and dispose infected animals. Therefore, in India, only a strategy based on vaccination is viable.<\/p>\n<p id=\"h-a-multidimensional-and-multidisciplinary-approach\"><strong>A multidimensional and multidisciplinary approach<\/strong><\/p>\n<p>The <a href=\"http:\/\/www.oneworldonehealth.org\/sept2004\/owoh_sept04.html\" target=\"_blank\" rel=\"noopener noreferrer\">\u201c12 Manhattan Principles\u201d<\/a> were presented in 2004 in New York (USA) at a conference organized by the Wildlife Conservation Society.<\/p>\n<p>The first of these principles stresses the need to recognise the links between human health, animal health and the environment. It also points out the need for holistic and prospective approaches to emerging infectious diseases considering the complex interconnections between species. Or, that of a reduction in trade of wild animals because of \u201cthe real threat it poses to global socio-economic security\u201d; increased investment in health infrastructure and infectious disease surveillance networks; quick and clear sharing of information; education and sensitisation of populations and political decision-makers to the interconnection of living things.<\/p>\n<p>The conclusion delivered in the congress summary is critical:<\/p>\n<figure id=\"attachment_6759\" aria-describedby=\"caption-attachment-6759\" style=\"width: 955px\" class=\"wp-caption alignright\"><img loading=\"lazy\" class=\"size-full wp-image-6759\" src=\"https:\/\/www.fie.undef.edu.ar\/ceptm\/wp-content\/uploads\/2020\/10\/paraguas.jpg\" alt=\"\" width=\"955\" height=\"730\" srcset=\"https:\/\/www.fie.undef.edu.ar\/ceptm\/wp-content\/uploads\/2020\/10\/paraguas.jpg 955w, https:\/\/www.fie.undef.edu.ar\/ceptm\/wp-content\/uploads\/2020\/10\/paraguas-300x229.jpg 300w, https:\/\/www.fie.undef.edu.ar\/ceptm\/wp-content\/uploads\/2020\/10\/paraguas-768x587.jpg 768w\" sizes=\"(max-width: 955px) 100vw, 955px\" \/><figcaption id=\"caption-attachment-6759\" class=\"wp-caption-text\">The \u2018One Health Umbrella\u2019 developed by the One Health Sweden and One Health Initiative to illustrate the scope of the One Health Initiative<\/figcaption><\/figure>\n<p>The <a href=\"https:\/\/www.oie.int\/doc\/ged\/D5720.PDF\" target=\"_blank\" rel=\"noopener noreferrer\">\u201cOne Health\u201d concept<\/a> was introduced in 2008 in Sharm el-Sheikh (Egypt), during a symposium on the risks of infection linked to contact between human and animal ecosystems.<\/p>\n<p>Shortly afterwards, following the <a href=\"https:\/\/en.wikipedia.org\/wiki\/2009_swine_flu_pa\" target=\"_blank\" rel=\"noopener noreferrer\">H1N1 influenza A pandemic of 2008-2009<\/a>, WHO adopted a <a href=\"https:\/\/www.who.int\/influenza\/resources\/pip_framework\/en\/\" target=\"_blank\" rel=\"noopener noreferrer\">global influenza program<\/a> involving increased surveillance of animal reservoirs. At the same time, the first <a href=\"https:\/\/www.cdc.gov\/onehealth\/index.html\" target=\"_blank\" rel=\"noopener noreferrer\">One Health agency<\/a> was created in the United States. It is now working to promote a <a href=\"https:\/\/ghsagenda.org\/\" target=\"_blank\" rel=\"noopener noreferrer\">global health security agenda<\/a>, in cooperation with many other national and international organisations, and involves around 60 countries.<\/p>\n<figure id=\"attachment_6760\" aria-describedby=\"caption-attachment-6760\" style=\"width: 920px\" class=\"wp-caption alignright\"><img loading=\"lazy\" class=\"size-full wp-image-6760\" src=\"https:\/\/www.fie.undef.edu.ar\/ceptm\/wp-content\/uploads\/2020\/10\/circulos.jpg\" alt=\"\" width=\"920\" height=\"886\" srcset=\"https:\/\/www.fie.undef.edu.ar\/ceptm\/wp-content\/uploads\/2020\/10\/circulos.jpg 920w, https:\/\/www.fie.undef.edu.ar\/ceptm\/wp-content\/uploads\/2020\/10\/circulos-300x289.jpg 300w, https:\/\/www.fie.undef.edu.ar\/ceptm\/wp-content\/uploads\/2020\/10\/circulos-768x740.jpg 768w\" sizes=\"(max-width: 920px) 100vw, 920px\" \/><figcaption id=\"caption-attachment-6760\" class=\"wp-caption-text\">Graphic representation of \u2018One Health\u2019. Wikipedia<\/figcaption><\/figure>\n<p>Since the emergence of \u201cOne Health\u201d, other <a href=\"https:\/\/www.frontiersin.org\/articles\/10.3389\/fvets.2017.00163\/full\" target=\"_blank\" rel=\"noopener noreferrer\">similar concepts have emerged<\/a>, such as <a href=\"https:\/\/en.wikipedia.org\/wiki\/EcoHealth\" target=\"_blank\" rel=\"noopener noreferrer\">EcoHealth<\/a> and <a href=\"https:\/\/en.wikipedia.org\/wiki\/Planetary_health\" target=\"_blank\" rel=\"noopener noreferrer\">Planetary Health<\/a>. Taking into account the interconnection of living things, but also the necessary abandonment of any linear and reductive thinking, and having in common a decompartmentalization of disciplines, these new concepts should make it possible to better understand and manage health crisis.<\/p>\n<p id=\"h-the-importance-of-anticipating\"><strong>The importance of anticipating<\/strong><\/p>\n<p>We all live in the same world, and both our health and our economy are closely linked to its condition. If we want to achieve the United Nations <a href=\"https:\/\/www.un.org\/millenniumgoals\" target=\"_blank\" rel=\"noopener noreferrer\">Millennium Development Goals<\/a>, it is imperative to consider the multiple interactions between public health, economy, animal health and the environment. However, as the <a href=\"https:\/\/www.rtbf.be\/info\/monde\/detail_pour-l-oms-beaucoup-de-pays-n-en-font-pas-assez-pour-combattre-le-coronavirus%C2%A0%C2%C2%A0%A0%C2%A0?id=10449010\" target=\"_blank\" rel=\"noopener noreferrer\">chaotic management<\/a> of the Covid-19 pandemic has highlighted, the \u201cOne Health\u201d vision is rarely adopted by policy makers.<\/p>\n<p>While the emergence of new viral pandemics of zoonotic origin has been <a href=\"https:\/\/www.nature.com\/articles\/nature12711\" target=\"_blank\" rel=\"noopener noreferrer\">repeatedly<\/a> <a href=\"https:\/\/www.nature.com\/articles\/nm.3985\" target=\"_blank\" rel=\"noopener noreferrer\">predicted<\/a> by the <a href=\"https:\/\/www.pnas.org\/content\/113\/11\/3048\" target=\"_blank\" rel=\"noopener noreferrer\">scientific community<\/a>, governments have been <a href=\"https:\/\/www.lemonde.fr\/sciences\/article\/2020\/06\/20\/richard-horton-le-covid-19-montre-une-faillite-catastrophique-des-gouvernements-occidentaux_6043590_1650684.html\" target=\"_blank\" rel=\"noopener noreferrer\">unable to anticipate<\/a>. Worse, they have put in place diverse (and sometimes contradictory) national measures, while a pandemic is clearly a global problem requiring concerted international action.<\/p>\n<p>Of course, we must learn from this. In the \u201cOne Health\u201d vision, the pandemic induced by <a href=\"https:\/\/en.wikipedia.org\/wiki\/Severe_acute_respiratory_syndrome_coronavirus_2\" target=\"_blank\" rel=\"noopener noreferrer\">SARS-CoV-2<\/a> is the direct and predictable consequence of the globalisation of animal production systems, the sale of live wild animals, but also of mass tourism, international trade and the hypermobility that accompanies them.
